CAGE Questionnaire

Is it possible that you need to seek help for your substance use? This is the first step in addressing your problem.
We provide an assessment and screening for you at your first appointment, along with a thorough review of your past history with alcohol and/or drugs. It will help give us the answers we need in order to offer you a specialized treatment plan. There is hope for anyone who has the desire for change.

Please take the CAGE Questionnaire below:

1.  Have you ever felt you should CUT down on your usage of alcohol (or drugs)?

3.  Have you ever felt bad or GUILTY about your drinking (or drug use)?

2.  Have people ANNOYED you by criticizing your drinking (or drug use)?

4.  Have you ever had a drink (or drug) first thing in the morning or to get rid of a hangover (EYE OPENER)?​

                   If you answered "yes" to any of the questions, please call our office for further testing and/or treatment.
